You are completing an electronic signature and understand that you are in accordance & agreement with the cancellation process. NO verbal, third-party, or other types of notifications will be accepted. I, said member, can be held legally bondable to my cancellation request, in accordance with Total Body Gym’s terms and agreement. (A copy can be found & printed on the website).

Cancellation Terms:

Our policy requires a FULL TWO-MONTH notice before cancellation is complete. By signing below, I agree to this policy and acknowledge that all required drafts and fees will be collected in accordance with my membership agreement. I also acknowledge that my access (key) tag remains the property of Total Body Gym and must be returned as outlined in the terms of cancellation.

Legal Agreement:

Total Body Gym will lawfully collect all fines, extra fees, and surcharges allowed by law through the agreed terms of service. All collection negotiations are conducted before the lawsuit is filed. You will receive an invoice or a notice of intent to file, which you may then dispute charges. Failure to respond, it is implied you accept all charges listed, and a lawsuit will be registered to collect payment. Any time after the litigation process begins or a lawsuit is filed, you will have Additional Legal Fees applied to your account. Abrupt stop payments and closed draft accounts are an immediate breach in terms of membership, & will file suit immediately upon receiving the notification or refusal of payment. I understand by submitting this request and NOT turn in my access (key) tag, I can be held liable for additional monthly membership prices because it is my implied consent to continue my membership as a cash-paying customer.
